Woodhaven Visitation Safety Guidelines

Effective: Friday, August 12th

Nursing Home safety guidelines from federal, state and local authorities are rapidly changing in response to the highly transmittable COVID-19 Delta and Omicron variant. Nursing home residents are considered the most vulnerable people in our country. We have had to adjust our visitation guidelines accordingly for the safety of our residents.

  • Only fully vaccinated visitors are allowed entry for visitation. (Unless special permission is obtained from administration.)

We understand a very small number of people can’t get vaccinated for medical reasons. Other than these rare exceptions, no unvaccinated visitors should be allowed entry to Woodhaven.

Acceptable proof of vaccination includes a COVID-19 Vaccination Record Card issued by the Centers for Disease Control and Prevention, a World Health Organization Yellow Card, or documentation from a licensed healthcare provider.

  • We ask that you contact the facility to schedule an appointment prior to arrival. (We do not want multiple visitors in the facility at the same time.)

  • 2 visitors per resident are permitted unless given permission by the nursing director.

  • Visitors will only be permitted to stay for one hour.

  • All visitors to the facility are required to sign the Visitor Log Book.

  • All Visitors will be required to wear a mask (surgical, cloth or other face coverings) during their visit. No exceptions.

If you do not feel well, if there is any chance you have been around someone who has tested positive for the Coronavirus, or have been in an area where an outbreak has occurred within the last 14 days, please stay home for the health and well-being of yourself and others.

  • Visits will be suspended if there is an active case of COVID-19 in the building.

  • Please understand these guidelines are subject to change based on CDC and local health department guidance.
